Patient Information

We welcome new and returning Capital Women’s Care patients. Our staff is committed to ensuring that the administrative aspects of your visit are handled with the same level of professionalism and care as the medical portion. Please bring the following to each appointment:
  • Current insurance card or insurance information
  • Driver’s license or photo ID
  • Insurance referral, if necessary
  • Co-payment (cash, check, Visa, MasterCard, and Discover accepted). If you are unable to make your payment, your appointment will be rescheduled.

Please arrive 20 minutes before

your scheduled appointment time to complete the necessary registration information.

Late Arrivals

If you are more than 15 minutes late for your appointment or we cannot complete the required administrative preparation for your appointment without delaying other patients, it may be necessary to reschedule your appointment. Every effort will be made to be sure you are seen in a timely manner.

Missed Appointments

Please cancel your scheduled appointment by phone at least 24 hours in advance. If you do not cancel within 24 hours of your appointment time, you may be assessed a $35 missed appointment fee. This fee is not covered by insurance carriers and it will be your responsibility to pay. Our aim is to open unused appointments for our patients, not to collect missed appointment fees. Your cooperation is greatly appreciated.

Emergencies

Obstetrics and gynecology often involve emergency procedures, surgical delays, and deliveries. We ask for your understanding, and if we are running behind schedule, our staff may attempt to notify you.

Insurance

We participate with many insurance companies. Please confirm the participation of our providers with your insurance company.

Prescription Refills

Feel free to call us for prescription refills during regular business hours. Be aware that refills may take up to 48 hours to be sent to your pharmacy. Prescription refills requested after hours and on weekends will be completed the next business day. In order for us to process your request, we need the name and dosage of the medication, pharmacy and location, and a phone number where we can reach you.

Records Release Policy

Capital Women’s Care maintains the patient’s rights wherein all medical records are kept confidential according to all federal and state laws. Federal and state law requires the patient’s signature, or that of an authorized personal representative, to release or transfer medical records. The patient records are defined as all records that relate to the health of the patient and are prepared by or under the supervision of a healthcare provider associated with Capital Women’s Care. Patients who provide records from another physician, outside of CWC, are encouraged to retain their own copy as Capital Women’s Care cannot provide a copy.
